Draft authorization form

Capture customer authority before branded labels, artwork, packaging, or relabeling work starts.

This draft supports FNSKU, UPC/EAN, barcode, brand label, sticker, insert, packaging replacement, and supplier-side relabeling workflows.

Fields to capture

The final form can live in customer onboarding and work orders after legal review.

Customer authority

  • Customer company, signatory, role, brand name, authorized sales channels, destination countries, version/date, and supporting document links
  • Confirmation that customer owns, licenses, or is authorized to use supplied marks, artwork, barcodes, labels, packaging, and product claims

Work scope

  • SKU list, label type, file source, approved quantity, destination, packaging rules, insert/card rules, before/after photo requirement, and work-order reference
  • Authority for PrepPort to print, apply, replace, remove, inspect, photograph, or handle labels and packaging within scope

Hold conditions

  • Missing or conflicting files
  • Unclear brand authorization
  • Marketplace or destination restriction
  • Mismatch between SKU, barcode, carton, or package instruction
  • Suspected counterfeit or IP-sensitive goods

Customer declaration draft

Legal should revise exact wording before use.

Authorization

Customer authorizes PrepPort to use the supplied files, marks, labels, barcodes, artwork, and packaging instructions only for the approved work order, shipment, SKU list, and sales-channel scope.

Responsibility

Customer remains responsible for trademark, marketplace, customs, destination-country, product-labeling, barcode, safety, and packaging compliance. PrepPort may pause work if authority or instructions are incomplete.

Approval questions

Resolve before adding the form to live onboarding.

Scope level

Should authorization be per account, brand, SKU, shipment, work order, or label file?

Proof needed

Which documents are required for branded work, and when is customer self-certification enough?

Status mapping

Which customer-visible status appears when authorization is missing, rejected, expired, or under review?

Registered mark symbol

Do not print, apply, or reproduce a registered trade mark symbol unless the customer confirms registration and authorizes its use for the approved scope.
